DFHV Complaint Form

Use this form to file a complaint with the Department of For-Hire Vehicles about a vehicle-for-hire trip in the District of Columbia. The Department of For-Hire Vehicles has jurisdiction over vehicles-for-hire which are licensed in Washington, DC. The Department of For-Hire Vehicles will investigate the incident and make a determination that could result in a fine or action against the driver or company. If the vehicle or company is not licensed in Washington, DC, please direct your complaint to the appropriate jurisdiction’s vehicle-for-hire licensing agency.

Additionally, if you also believe you were discriminated against by a driver or company while in the District of Columbia, you can also file a complaint with the DC Office of Human Rights by completing Section II of the form. The DC Office of Human Rights will initiate its own complaint procedure. More information can be found at: http://ohr.dc.gov/taxis.
